Summary

Description
English: Replication of flight conditions on the accident flight of G-YMMM using a fuel system mock-up revealed widespread systemic ice accumulation. The ice did not affect performance until it was released at warmer temperatures and under higher fuel flow than experienced during cruise. The released ice gathered at the fuel-oil heat exchanger (FOHE) on each engine, where it obstructed fuel flow to the engines, causing them to fail.

Captions

Ice accumulation in a fuel system test rig used to investigate the crash of BA Flight 38. This ice accumulation did not cause the failure. It was when this ice was released at warmer temperature and higher fuel flow that it clogged the FOHE.
